| Mission Statement |
| The purpose of this foundation is to generate, accept, and distribute funds for education
and scientific endeavor for the betterment of nursing students. Funds will be awarded for but
not limited to scholarships and workshops in the field of nursing.

| Scholarship Application |
| |
| The Student Nurses' Foundation of Wisconsin will be offering several scholarships
up to $500 each. The scholarships are offered to all WSNA members, except WSNA board
members and Foundation Directors.

| Eligibility |
| Students who are members of WSNA and will be undergraduate students during the next
academic year. Selection will be based on professionalism, academic achievement, financial
need, and recommendations. |

| Instructions |
- Proof of membership in NSNA/WSNA: a copy NSNA membership card, cancelled check, or receipt.
- Copies of nursing transcripts or grades and all recent college transcripts.

The entire application and references must be completed and received no later than 9:00am on Saturday
morning of the Annual WSNA Convention. Only completed applications will be considered.

| Selection and Notification |
| All scholarship applications will be judged confidentially and impartially by the
Directors of SNFW. Winners will be announced at the WSNA Annual Convention. |
